The Practice runs Emergency Admissions and Risk Stratification Queries on a six monthly basis. Both Query extracts are shared with HSCB in pseudonymised form. The practice compares the results of the Admissions Query with admission numbers from PAS data. The practice then reflects on patients admitted compared to those previously identified as being at high risk of admission. Patients should be considered in three groups: (1) high risk admitted, (2) low risk admitted and (3) high risk not admitted to enable discussion at Peer Review meetings
