Minimising the harms of polypharmacy. The contractor will: 1. Identify and record number the % of patients aged 85 years or more receiving 6 or more medications. 2. Undertake face to face medication reviews, using the “No Tears“ approach or similar tool as agreed within the cluster, for at least 60% of the cohort defined in 1 above (for a minimum number equivalent to 5/1000 registered patients. If the minimum number of reviews cannot be undertaken because of the small size of the cohort defined in 1 above, consider reducing the age limit until the minimum is reached). 3. Identify actions to be addressed in the Practice Development Plan. 4. Summarise themes and actions for review with the GP cluster network and share information with the LHB as required.
