Improving end of life care. The contractor will: 1. Identify all deaths 170(up to a maximum of 5/ 1000 registered patients) occurring between 1st January 2014 and 31st December 2014. 2. Use the individual case review to assess delivery of end of life care (at Appendix 2, Annex 7). 3. Identify and include actions to be addressed in the Practice Development Plan. 4. Summarise themes and actions for review with the cluster network at the meetings and share information with the LHB as required.
